Job Summary:

Wayne State University Physician Group, one of Southeast Michigan’s largest multi-specialty Physician Practice Groups is currently seeking a Nurse Practitioner to work within our Department of Internal Medicine for our Hospitalists, located in Detroit. This is a position working full time on nights.

Candidate will obtain and record patient medical history; performs physical examinations; orders, evaluates and monitors medical and laboratory diagnostic tests and treatment plans. Prepares, develops and initiates treatment plan in consultation with attending physician; performs routine invasive procedures; performs diagnostic and therapeutic procedures. Educates and counsels patient and family members on condition, desired results of treatment plans, expectations during illness and recovery, and other related areas. Participates in the management of patient's care, determining need for consultation to ensure the continuity and quality of care.

Must have high level of interpersonal skills, written and verbal, necessary to effectively communicate, establish, and maintain productive working relationships with people inside and outside the Department and/or Practice.

Excellent analytical ability necessary to independently identify and resolve extremely complex and unusual problems or discrepancies.

Position requirements:

  • Graduate of an accredited Acute Care, Adult Primary Care, or Family NP program, with ANCC or AANP certification.
  • Registered Nurse licensure to practice in State of Michigan.
  • Master’s degree in nursing with clinical specialization.
  • Supervisory experience and ACLS are preferred.

Job Summary:

Residential Physicians Association is seeking 3 Nurse Practitioners to join the "RPA CARES TEAM". NP will conduct monthly home visits to our new and current patients in their home or current residence. The assessment will cover a full medical history and medication review as well as a physical to capture a comprehensive picture of the patient's health status, including the supporting evidence for all medical conditions. "THE RPA CARES TEAM" excels at delivering high-touch, high-value assessment and care management services for its patients in a 5 county area. We are known for our successful in-home care and well established chronic care management team. We have provided in home and in facility personalized care for over 25 years and can use your help.

Responsibilities:

  • Driven to 10 plus appointments per day by our highly trained medical assistants in our company fleet cars.
  • Identifies opportunities for health promotion and illness prevention.
  • Complete all documentation from visit while traveling or on a timely basis.
  • Communicate clearly and timely with manager when a clinical escalation is found.
  • Document clinical escalations appropriately.

Qualifications:

  • Advanced practice degree and current Nurse Practitioner license in Michigan
  • Maintains current CPR certification
  • Nurse Practitioner Board certification preferred
  • Strong professional level of knowledge and comprehensive clinical assessment skills in the adult population and chronic disease management
  • Excellent computer skills - familiar with Microsoft Office and comfortable utilizing a laptop to document
  • Must be willing to multi task and deal with chronically ill patients who can often be difficult.
